State agencies only - This Website provides the First Report of Injury Form (and related instructions) that is to be used ONLY by Minnesota state agencies to document job-related illness/injuries sustained by state employees.

Minnesota employers - All other Minnesota employers should use the First Report of Injury Form issued by the Minnesota Department of Labor and Industry (found at http://www.dli.mn.gov/pdf/effroi.pdf ) when documenting such employee illness/injury.

The State Workers' Compensation Program is a self-insured, self-administered program that covers approximately 54,000 employees of the executive, legislative and judicial branches of state government; along with employees of some quasi-state agencies such as the State Fair and the Minnesota Historical Society. The program provides or coordinates claims management, disability management and managed health care for state workers who are injured on the job; and offers workers' compensation legal services for state agencies.

The First-Report-of-Injury/Claims Management Resource is designed to guide state agency supervisors, Workers’ Compensation Coordinators, GenComp users, and Human Resource Office personnel in the following areas:

  •  Proper preparation and filing of a First Report of Injury (FRI) for state employees who have been injured on the job.

  • Proper entry of FRIs into the SEMA4 system.

  • Proper navigation of the State Workers’ Compensation Program’s GenComp system.

Important note for state workers who need health care for treatment of job-related injuries:
CorVel is the certified managed health care plan under contract with the State Workers’ Compensation Program to cover all state employees for work-related injuries and illnesses. Employees may call (612) 436-2542 or (866) 399-8541 to speak with a nurse about their work-related injuries and illness; and to receive information about CorVel's provider network. 

The  Disability Management Unit manages disability claims by working with employees, state agencies, workers’ compensation specialists and other professionals to help injured state workers stay on the job or return to work quickly.

The State Safety & Industrial Hygiene Unit provides state agencies with consultation, training and assessment services that assist with development of policies/programs designed to prevent job-related injury/illness among employees.
